Birth certificates for anyone under 18.
2 Months of most recent pay stubs(for everyone in the household 18 years or older)
Tax returns and W2s for previous 2 years (2021/2022). If self employed, then 1099s
3 months of most recent financial statements (checking/savings/retirement or investment accounts)-all pages
Award letters - for social security/disability or retirement
If applicable, proof of child support and/or divorce decree
If applicable, bankruptcy papers (including discharge letter and list of creditors)
